Qualifications for home healthcare services:
Individuals must be ‘homebound’, meaning you have difficulty leaving your home without assistance and in need of skilled medical services due to:
- Follow-up care after returning home from a nursing facility, rehabilitation center, or hospital
- Recovery from a recent illness, injury or surgery
- Management of a chronic health condition
- A need for any other specialty services
Home healthcare services include:
- Rehabilitation or orthopedic services
- Fall prevention
- Wound care
- Ostomy management
- Chronic care management
- Diabetes care and education
- Oncology services
- Telehealth monitoring
- Medication management
Home healthcare team members include:
- Registered Nurses, Licensed Practical Nurses
- Physical Therapists and Physical Therapy Assistant
