We are seeking a dedicated and compassionate Physical Therapist to join our team. The ideal candidate will have a strong background in patient care and be skilled in developing effective treatment plans tailored to individual patient needs. The Physical Therapist will play a vital role in improving patients’ mobility and quality of life through evidence-based practices.
We are a Christian-based, clinician-led home health agency proudly serving 10 counties in South Carolina. Founded by a licensed Physical Therapist and MUSC graduate, our mission is to provide high-quality patient-centered care that promotes health improvement, independence, and holistic well-being for patients across South Carolina. We are deeply rooted in our community, and we’re looking for skilled, mission-driven Physical Therapists to join our growing team.
• Deliver in-home physical therapy services to patients in your community (New company care provided)
• Create individualized care plans to promote independence and function
• Work collaboratively with nurses, caregivers, and our interdisciplinary team
• Educate patients and families for continued care and mobility
• Ensure documentation meets CMS and ACHC accreditation standards
• Conduct comprehensive assessments of patients to determine their physical capabilities and limitations.
• Implement therapeutic interventions aimed at improving mobility, strength, and overall physical function.
• Monitor patient progress and modify treatment plans as necessary to ensure optimal recovery outcomes.
• Collaborate with other healthcare professionals to coordinate discharge planning and ensure continuity of care.
• Educate patients and their families on exercises, home care strategies, and preventive measures to enhance recovery.
Experience
• At least one year of previous home health experience preferred
• Excellent communication and time management skills
• Minimum of one year previous experience in home health required
• Strong knowledge of medical terminology and documentation practices is essential.
• Experience with developing care plans that align with service-driven best practices
• Ability to work effectively within a multidisciplinary team while maintaining a patient-centered approach.
• Heart for service and a desire to make a difference
• Reliable transportation and ability to travel locally.
