Coastal Health’s home healthcare service offers a full range of skilled nursing and therapy care to meet the needs of patients in their own homes as they recover from short-term illness, injury, surgery, or manage a chronic health condition.

Qualifications for home healthcare services:

Individuals must be ‘homebound’, meaning you have difficulty leaving your home without assistance and in need of skilled medical services due to:

  • Follow-up care after returning home from a nursing facility, rehabilitation center, or hospital
  • Recovery from a recent illness, injury or surgery
  • Management of a chronic health condition
  • A need for any other specialty services

Home healthcare services include:

  • Rehabilitation or orthopedic services
  • Fall prevention
  • Wound care
  • Ostomy management
  • Chronic care management
  • Diabetes care and education
  • Oncology services
  • Telehealth monitoring
  • Medication management

Home healthcare team members include:

  • Registered Nurses, Licensed Practical Nurses
  • Physical Therapists and Physical Therapy Assistant
