Qualifications for home healthcare services:

Individuals must be ‘homebound’, meaning you have difficulty leaving your home without assistance and in need of skilled medical services due to:

Follow-up care after returning home from a nursing facility, rehabilitation center, or hospital
Recovery from a recent illness, injury or surgery
Management of a chronic health condition
A need for any other specialty services

Home healthcare services include:

Rehabilitation or orthopedic services
Fall prevention
Wound care
Ostomy management
Chronic care management
Diabetes care and education
Oncology services
Telehealth monitoring
Medication management

Home healthcare team members include:

Registered Nurses, Licensed Practical Nurses
Physical Therapists and Physical Therapy Assistant
